How to register for Short Breaks
To access any NH&SCVS Breakaway service children and young people must be registered for Short Breaks on the Hertfordshire County Council's Short breaks local offer (SBLO) online portal.
If you are currently receiving any support from Hertfordshire's 0-25 Together Service please contact the Brokerage team on 01438 844254 to check eligibility.
1 Complete your SBLO Account application - please see link below.
2 Email breakawayadmin@nhcvs.org.uk to register your interest.
3 You will then be invited to visit playscheme at Greenside school where you can see if the scheme looks suitable for your child, and where you will be given an application form and care plan form to read.
4 We will arrange to visit you and your child at home, and together we will complete the application form, care plan and check your documentation.
5 Once all the paperwork is complete you will be able to 'Request a place' for your child via our website.
Please remember that all applicants must also have registered on the SBLO online portal.
The SBLO online portal allows you to register your interest in short break activity providers; renew your application; update your details; see the total number of hours you have remaining/used.
Your account will show the full annual hours you are entitled to. The SBLO year runs from 1 April to 31st March.
